Drug Test Consent Form

If you are offered and accept employment with Manco & Manco Pizza®, in the interest of safety for the company, co-workers, clients, customers, and anyone else you may have contact with because of your employment with us, you may be required to submit to a random urinalysis test for illegal drug use. This test will be paid for by the employer. The results collected by the lab will be sent to Universal Background Screening.
I, the undersigned, have been fully informed of the reason for the test for illegal drugs, I understand what I am being tested for, the procedure involved, and do hereby freely give my consent. In addition, I understand that the results of this test will be forwarded to my potential employer Manco & Manco®, by the participating drug testing company (LabCorp or Quest Labs), and will become part of my file. If this test is positive, and for this reason, I am not hired, I understand that I will be given the opportunity to explain the results of this test and or provide a doctor's ordered prescription for the said drug, such as opioids. Cash Transaction Test

The position that you are applying for requires you to perform a substantial amount of quick, but more importantly, ACCURATE cash transactions.

When used correctly, our POS system will display the appropriate change owed to the guest. There will be occasions, however, when the system may be unavailable due to a power outage or other technical issues. As someone who will be handling cash on a regular basis, you must have the ability to perform quick, ACCURATE transactions, even if the system is down.

You will have 7 minutes to complete 10 questions based on actual transactions that you may encounter.

CALCULATORS, PHONES, ETC ARE NOT PERMITTED. Test scores have no effect on interview opportunities. Applicants will be required to take a similar test during the interview process.
