- I understand no child under the age of 11 is permitted at the Campout events, and I have not purchased passes for anyone under the age of 11. I understand anyone attending who is 11 years old must also have completed Grade 6.

- I understand the Group Leader identified on this form will be the primary contact for their group.

- I understand the Group Leader is required to complete all required forms for the event registration.

- I understand there are no refunds for any reason.

- I understand attending Ultimate Summer Campout implies consent to be photographed, and photos taken during this week may be used to promote events in other Maritime PAOC publications and/or promotions in print and online.
