MARLEIGH'S FRIENDS cannot guarantee that any dog is 100% potty trained or will be the perfect match in your home, but will do everything possible to find the best fit. I understand that any dog must have time to decompress in a new environment and I am willing to be patient and understanding while helping this dog(s) adjust to these changes.

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any MARLEIGH'S FRIENDS animal adopted or fostered by me.

I certify that the above information is correct, and I understand that the information will be verified.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless MARLEIGH'S FRIENDS and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the dog.

I accept full responsibility for the dog(s) actions at all times, and release MARLEIGH'S FRIENDS from any liabilities or damages that may incurre.

I agree to have MARLEIGH'S FRIENDS complete reference call checks and conduct a home visit inspection to be able to approve my application.

I agree that if, for any reason, I'm unable to keep or provide for the dog(s) anymore that I will return the dog(s) to MARLEIGH'S FRIENDS and try to give MARLEIGH'S FRIENDS a 2 week period to try and find another suitable home for the dog(s) in my care.

I also understand that any adoption "fee" to MARLEIGH'S FRIENDS is a tax deductible donation and it is nonrefundable.
