My child has my permission to attend the 2026 Ohio Fair Managers’ Conference in Columbus Ohio on January 17, 2026. I understand that we are to meet at the Canfield Fair Office on the Canfield Fairgrounds prior to 5:00 a.m. I understand the bus will leave promptly at 5:10 a.m. I understand that the trip will be supervised by Junior Fair Board Advisors, Junior Fair Committee members and the Manager of Junior Fair. The youth will be traveling by Charter Bus to and from the Canfield Fairgrounds. In case of emergency, I give my permission for emergency medical treatment to be administered to my child with the consent of the Manager of Junior Fair and/or Junior Fair Board Advisor, per the signed Medical Information and Release Form.
