Before you upload, please check your child's Junior Preliminary Evaluation form has all of the following:
- Child's name — full name, as registered with the club
- Age group — U6 through to U14
- Task Complete/Comments — signed or initialled in the box beside your child's age group row, so we can see which tasks were assessed
- Swim coach's name
- Australian Swimming Teachers and Coaches Association number
- Swim coach's signature
- Date witnessed
Forms submitted without all of these details will be returned to you for resubmission.
