Faith Formation Family Enrollment Form
  • Dear Parent or Guardian,

We are excited for you and your entire family to join us for the 2025-2026 faith formation programs at St. Thomas Aquinas. This new year, we are using a family-focused, church supported approach to faith formation, sacramental preparations and evangelization.

Please note that moving forward and in alignment with the Archdiocese of Denver's requirements for confirmation, sacramental preparations at St. Thomas Aquinas will require 2 years of formation for the Sacrament of Confirmation.

This enrollment form is for the whole family, for students of all grades K through 12th Grade, for the academic year and it includes preparations for the Sacraments. Additional paperwork may be required for sacrament preparation classes.

Though this is “just an enrollment form”, our desire is to simplify your parent work effort and demonstrate our renewed efforts to minister to and with the whole family.

All communications are sent through Flocknote, therefore an up-to-date family email address and phone number are necessary for effective communication of class details and information.

If you have questions or concerns, please contact our office at 303-443-8383.

  • Volunteer Opportunities

  • Companions: We couldn't provide these programs without the help of willing adults to lead and show up for the children.  We are in need of a team of volunteers who can help lead small group discussions and walk with the families.  Are you or a family member interested in being a part of the companions team as a volunteer this year?
  • If yes, is the family member Safe Environment Trained within the last 5 years?
  • Intercessors: Prayer is the most powerful gift we could give our families! We are in need of volunteers who are willing to consistently pray for our program, leaders, and families. Are you or a family member interested in being a part of the prayer team?
  • Hospitality: We are in need of a group of volunteers who are willing to help set up the sessions with the necessary items. Are you or a family member interested in being a part of the hospitality team?
  • Family Commitment to the Faith Formation Program

Below are policies that families agree to abide by as part of partaking in this program.
  • In addition to being the primary educators in the faith, parents by their enrollment into the Faith Formation agree to support the process in the following ways:

Domestic Church: It is important to recognize that the cathecists assist the domestic church (your household) in training the children in the faith; they cannot replace you, nor are the At-Church sessions sufficient to fully pass on the faith to the children.  Please walk with your child every step of the way and be sure to share the beauty of the faith with them on a daily basis.

In-Home Meetings: The Pathways Foundation Suite curriculums require that the domestic church continue faith conversations inside the home with In-Home meetings.  Please follow along the class schedules and plan your weeks to include an In-Home meeting between At-Church sessions.

Parent Attendance: A parent or guardian MUST attend the parent session every time the child is in a children’s session. This is not a drop off catechesis program. Furthermore, since building community is an important aspect of the Family Faith Formation Program it is best if the same person or persons can be present for the sessions.

Safe Environment Certified: If not already safe environment certified, parents agree to ensure that one adult in the family will become safe environment certified by the Archdiocese of Denver within 6 months of enrollment. This will allow families to participate in the volunteer opportunities. You can find Safe Environment classes here: VIRTUS® Online

Attendance: Attendance is important for all families as it provides consistency and prioritizes the faith over worldly things.  For children in Sacramental Preparation, absences may result in a delay of receiving sacraments.  In addition, all in-home & at-church meetings and retreats are mandatory. Please contact a department leader when your child misses a class for the make-up.

  • Media Release: There may be times when photos or videos of your family members may be taken and possibly used to promote the Family Faith Formation through the church and/or social media during the year. I hereby give my permission for St. Thomas Aquinas and/or it's representatives to use photographs or videos of my child(ren) or myself. I understand that these may be used to inform families, volunteers, media and the general public about St. Thomas Aquinas and its programs, services or events. I gladly give this authorization to support the efforts of St. Thomas Aquinas and the Family Faith Formation program*
  • Program Fee

Due to the nature of the program, there is a cost associated with the purchasing of the materials necessary for the program to be successful.

We ask that each family help cover the cost of the materials necessary by paying the program fee according to the number of children that will be in the sessions K to 12th Grade.
