- How big is the problem, and who does it affect?
- What is currently being done about the problem, and why isn't it working? Provide some context.

- Proposed solution: Describe your idea in a brief, succinct, and clear statement. (Max 1-2 sentences)
- Impact: Describe the overall impact that you hope to achieve and how you will measure it.
4. Stakeholders and Resources (max 250 words)
- Who will you need to present your solution to to get support for and begin implementing your solution?
- What resources is your institution/organization providing to support your solution? (Examples: effort, funding, supplies, etc.)
- What support do you anticipate you will need? What barriers will you anticipate?
5. Budget (Please enter the amount requested. Only ask for the amount you need.)

6. Team Members: Please upload a document with your team members, their credentials, and titles.
- Tracks 1 & 2 Teams must be interdisciplinary and have at least one member of executive leadership as an advisor or sponsor to implement innovations within the health system/organization (Examples: Chief Nursing Officer, Chief Executive Officer, Chief Finance Officer). If selected as a finalist, you will be required to submit a letter of support from executive leadership and documentation outlined in the Funding Resource Guide.
