This form is being provided in order to notify you of Troop 12’s intention to place media that may include your Scout on the Troop 12 website, Facebook page and/or other Troop 12 digital properties. We will abide by the following rules and parameters in this process:

• Troop 12 will NOT include names, addresses or any other personal identification along with these pictures/videos.
• The pictures/videos will be for the exclusive use of promoting Scouting activities in general and Troop 12 in particular.
• Should you wish to have a picture/video removed from our website or promotional materials, Troop 12 will, in good faith, make every effort to do so in a timely manner, but we cannot guarantee immediate compliance with your request.

Please indicate your consent or refusal regarding the use of your Scout’s picture on the Troop 12 website and social media sites in the form below.

BY CONSENTING YOU AGREE:

I hereby assign and grant to the Boy Scouts of America the right and permission to use and publish the photographs/film/videotapes/electronic representations and/or sound recordings made of my Scout by Troop 12, BSA, and I hereby release the Boy Scouts of America from any and all liability from such use and publication. I hereby authorize the reproduction, broadcast, electronic storage and/or distribution of said photographs/film/videotapes/electronic representations and/or sound recordings without limitation at the discretion of Troop 12, BSA and I specifically waive any right to any compensation I may have for any of the foregoing.
