• Environmental Sensitivity Index (ESI) Map Request Form

  • To aid in processing your request, please complete the form below in its entirety.

    A minimum of 10 business days is required for all requests.

  • ENVIRONMENTAL SENSITIVITY INDEX (ESI) MAP REQUEST PROCESS

     

    1)   Complete and submit the ESI Map request form (online).

     

    2)   Agency to respond to the request stating appropriate charges based on the format of map requested and number of coverage sheets.

     

    3)    Company to make payment via https://www.remita.com and submit payment evidence along with the completed form to the Agency.

     

    4)   Upon confirmation of payment, ESI Map of the facility will commence and possible collection date will be communicated.

     

    7)   Map collection and sign off document endorsed.
