This form is for interest in a Registration Night for incoming PreK and Kindergarten students at Nettleton Public Schools. Please fill out the form if you are intested in participating. 

Required Documents for Registration

1. Three Proofs of Residence
   • Utlity bill/deposit receipt including street address
   • house or rental/lease contract
   • any current bill with your name and address
2. One Proof of Child's age
   • Birth Certificate
   • Passport
   • Hospital Record
3. Certificate of Immunization
   • Form from a licensed physician or public health department
   • Exemption form
4. Student Identification Number
   • Social Security Card
   • Copy of the parent/guaridan's drivers license or ID
   • Students insurance card
5. Document showing physical exam within the last 12 months
6. For PreK ABC Applications ONLY
   • Proof of income for every adult in household is needed
     • 30 consecutive days of pay
     • W-2
     • Lastest income tax return
   • No-Earned Income statement form if unemployed (Notarized)
   • School Schedule, if attending.
