    I hereby agree to abide by all rules and regulations governing the Games, as well as the decisions of the Games’ officials and organisers.

    I grant full permission to the Ministry of People Empowerment and Elder Affairs to use my likeness, name, voice, and words—during the event and at any time thereafter—across all media platforms, including but not limited to television, radio, film, newspapers, magazines, and digital platforms. This permission extends to uses for promotional purposes, public information, and applications for funding in support of the Games and their objectives.

    To the best of my knowledge and belief, I declare that I am physically and mentally fit to participate in the Games. I further confirm that I have been examined and certified as fit to participate by a licensed medical practitioner, and that no medical condition exists which would prevent my involvement in the event.

    I understand and accept that participation in the Games involves inherent risks. Accordingly, I hereby release and hold harmless the Ministry of People Empowerment and Elder Affairs, event organisers, sponsors, officials, and all affiliated personnel from any and all liability for injury, loss, or damage, howsoever caused, that may arise from my participation in the 2026 National Senior Games or from my presence at any official venue.

    In the event that I require emergency medical treatment during my participation and am unable to provide consent due to the nature of my injuries, I hereby authorise the Ministry or its representatives to take any necessary action, including transportation and hospitalisation, to safeguard my health and wellbeing.
