Assumption of Risk and Release of Liability:
I, the undersigned participant, understand that participation in basketball training activities conducted by Run The Show Enterprise LLC involves inherent risks, including but not limited to physical injury, illness, or property damage. These risks may arise from my own actions, the actions of others, or the conditions of the facilities or equipment used during the training sessions.
In consideration of being allowed to participate in the basketball training program offered by Run The Show, I agree to assume all risks associated with participation. I acknowledge that I or my child is voluntarily participating in this program and that I have been informed of the nature and extent of the risks involved.
Waiver and Release:
I hereby release, discharge, and hold harmless Run The Show Enterprise LLC, its directors, officers, employees, agents, and representatives, from any and all claims, demands, causes of action, liabilities, costs, and expenses, whether arising in tort or in contract, known or unknown, arising out of or relating to any personal injury, illness, or property damage that may occur as a result of my participation in the basketball training program.
I understand and agree that this release includes any claims based on the negligence, action, or inaction of Run The Show Enterprise LLC, its directors, officers, employees, agents, and representatives, whether a COVID-19 or other communicable disease exposure occurs or is alleged to have occurred before, during, or after my participation in the basketball training program.
Medical Authorization:
I authorize Run The Show Enterprise LLC and its authorized representatives to seek medical treatment for me in the event of an emergency if I am unable to do so myself. I agree to assume all costs associated with any necessary medical treatment.
Photography and Video Release:
I grant Run The Show Enterprise LLC and its representatives the right to take photographs and videos of me during the basketball training program. I understand that these photographs and videos may be used for promotional or educational purposes, including but not limited to websites, social media, and marketing materials.
I waive any right to inspect or approve the finished photographs or videos and acknowledge that I will not receive any compensation for their use.
Agreement:
By agreeing to this, I acknowledge that I have read this liability waiver form, understand its contents, and agree to be bound by its terms. I affirm that I am of legal age and competent to accept this agreement or that my parent or legal guardian has provided their consent.
