Consent and Liability Waiver/Social Media Waiver
By registering for and participating in the Northern Michigan Warhounds Prospect Camp, I acknowledge and accept that participation in hockey-related activities involves inherent risks, including but not limited to injury, illness, or property damage. I voluntarily assume all risks associated with participation and agree to release, waive, and hold harmless the Northern Michigan Warhounds organization, its owners, coaches, staff, volunteers, and affiliated facilities from any and all liability arising from my involvement in this camp.
I further grant permission to the Northern Michigan Warhounds to photograph, video record, and otherwise capture my likeness during camp activities. I understand that these images and recordings may be used for promotional purposes, including but not limited to social media, websites, advertisements, and other marketing materials, without compensation or further approval. By participating, I consent to the use of my name, image, and likeness in connection with these efforts.
By signing below, I confirm that I am at least 18 years of age and physically able to participate in hockey activities. I have read and fully understand this waiver and release, and I agree to its terms voluntarily.
