PLEASE REVIEW THE INFORMATION BELOW. PLEASE COMPLETE ONE APPLICATION PER CHILD. OCC Santa Project serves only residents of the Town of Ossipee with children, from newborn to 16 years of age. We do not serve unborn children and may request a Birth Certificate if necessary. Only parents/guardians can fill out an application on behalf of the family/child(ren).If you need assistance filling out this form, please contact a school liaison or OCC Staff to help guide you. We require a copy of the Parent/Guardian Photo ID (front and back), Proof of Residency and Proof of School Enrollment (if applicable) for each child with any application. Please see the correspondent sections for details on what is acceptable documentation. Each child gets one bag of gifts, each bag consists of 4 items (3 needs and 1 special wish). All 4 items TOTAL must be under $150; and gift card requests are NOT eligible.Please keep special gifts reasonable, as expensive electronics/items (i.e. iPhones, tablets, XBox, UGGs, etc) will result in us deleting your special gift request, meaning your child will get one less gift. Only one special wish item is allowed. "Double-dipping" between holiday programs, falsifying information, or submitting duplicated information will result on permanent ban from the program. We cross-reference information with other local programs to ensure fairness for everyone involved. IMPORTANT: Submitting an application does not guarantee approval. Our team will review your application, and contact you via email or text if additional information is needed to process your application. Once your application has been APPROVED, you will receive an email confirmation with your Child’s Tag Number. If your application is REJECTED, you will receive an email with information on the reason for rejection and will be given an opportunity to resubmit.
