TATTOO CLIENT CONSENT FORM

Welcome to Off-Gray Studio! 

Completing this consent form is not just a formality; it's an important step in ensuring your safety, well-being, and the success of your tattoo experience. This information helps us understand your needs, ensure compliance with legal requirements, and tailor our services to your unique preferences.

Your trust in us is invaluable, and we are honored that you've chosen to work with us.

Please fill out the following information as correctly and completely as possible. Fields required are marked (*),  all others are to your preference.

I, {nameas25},  acknowledge that this form will be placed under my client profile and will be used for all tattoos, including the one I am receiving as of the date mentioned above, as well as all future tattoos.

I have had the full opportunity to ask any questions I might have about receiving a tattoo from Off-Gray Studio and the artist listed above. I confirm that all my questions have been answered to my full and complete satisfaction.

I have been provided with information regarding the facts and matters outlined below and hereby indicate my agreement with my signature:

• I acknowledge that I have truthfully represented to all agents and representatives of Off-Gray Studio that I am at least eighteen (18) years of age and have presented the required state or federally issued picture I.D.
• I am not under the influence of alcohol or drugs, prescriptions or otherwise.
• I am free of communicable diseases, and I have made my artist aware of any health problems that I may have, including health problems that could affect the healing of this tattoo.
• I do not have a medical condition that would prohibit the service of a tattoo.
• I do not have skin conditions such as, but not limited to, acne, scarring/keloid, eczema, psoriasis, freckles, moles, or sunburn. If I have any type of skin condition, I understand that this may alter the outcome of my tattoo and do not hold the artist or Off-Gray Studio, LLC responsible. I will also notify my artist if I have any type of infection or rash anywhere on my body.
• I am not pregnant or nursing.
• I am not taking any medications/drugs that may affect the service of tattooing, such as, but not limited to, blood thinners, anti-rejection medication pertaining to an organ transplant.
• I acknowledge that infection is always possible as a result of obtaining a tattoo, particularly if I improperly care for my tattoo.
• I have received aftercare instructions and agree to follow them while my tattoo is healing. I recognize the absolute necessity of following these instructions.
• I agree that any touch-up work needed due to my own negligence will be done at my own expense.
• I acknowledge that it is impossible for the artists and any/all representatives of Off-Gray Studio to determine whether I may have an allergic reaction to the pigments, materials, or process used in the application of my tattoo. I agree to accept the risk knowing that such a reaction is possible.
• I acknowledge that obtaining a tattoo is my choice alone. I consent to the application of the tattoo and to any actions/conduct of the artists and any/all representatives of Off-Gray Studio.
• I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo.
• To my knowledge, I do not have a physical, mental, or medical impairment or disability that might affect my well-being as a direct or indirect result of my decision to have a tattoo.
• I acknowledge that variations in color and design may exist between any tattoo I select and its actual physical appearance on my body depending on my skin tone and texture.
• I release and forever discharge and hold harmless any and all members of Off-Gray Studio, from any and all matters, liabilities, claims, damages, actions, and demands in law or in equity arising from, or connected in any way to my tattoo or the procedures and conducts used to, and arising from, the application of my tattoo, by reason of complying with my request for this application.

I hereby authorize Off-Gray Studio to use my photo and/or information related to my experiences with Off-Gray Studio.

I understand that this information may be used in various publications, including electronic publications (i.e. social media), audiovisual presentations, promotional literature, advertising, community presentations, letters to area legislators and media, and/or other similar promotional materials.

My consent is freely given as a public service to Off-Gray Studio, without expecting payment. In exchange for the use of my photo and/or information, I release Off-Gray Studio and their respective employees, officers, and agents from any and all liability that may arise from the use of such news media stories, promotional materials, written articles, videotapes, and/or photographs.

I also acknowledge that I retain the right to revoke this authorization at any time, provided that I do so in writing and deliver it to Off-Gray Studio. Revocation of this authorization will not affect any actions taken before Off-Gray Studio receives my written notice of revocation.

For your safety and peace of mind, we request that you provide the name and contact information of an emergency contact. While not mandatory, it is highly recommended. In the unlikely event of a medical emergency during your tattooing session, this information will allow us to quickly reach out to someone who can provide vital medical history or make important decisions on your behalf. Rest assured that your privacy will be respected, and this information will only be used in case of emergency.

Your well-being is our utmost priority, and including an emergency contact helps us ensure a safe and responsible tattooing experience.
