Application for Membership

I, the undersigned, make application for admission to membership in the Office and Professional Employees International Union, AFL-CIO, Local 153, to serve as my chosen and authorized collective bargaining representative on matters relating to wages, benefits, hours and other working conditions of employment.

If accepted: "I pledge my honor to faithfully comply with the Constitution, Laws, and all amendments thereto, of Office and Professional Employees Union, Local 153, and of the International Union. I further promise not to divulge or make public any of the private proceedings of this Union, to faithfully perform all the duties assigned to me to the best of my ability and skill; to so conduct myself at all times as not to bring reproach upon my Union; and at all times to bear true and faithful allegiance to the Office & Professional Employees International Union of the AFL-CIO.”

I hereby assign to Local 153, Office and Professional Employees International Union, AFL-CIO, (“OPEIU Local 153”) from any salary or wages earned or to be earned by me as an employee of my Employer such sums as OPEIU Local 153 certifies as either membership dues, initiation fees, reinstatement fees, assessments or agency fees. I authorize and direct my Employer to deduct such amounts from my pay, during each month of my employment as a non-supervisory employee, and to remit the same promptly to OPEIU Local 153, in accordance with the collective bargaining agreement between OPEIU Local 153 and my Employer.

This assignment, authorization and direction shall be irrevocable for the period of one (1) year from the date of my signature below, or until the termination of the collective bargaining agreement between the Employer and OPEIU Local 153 which is in force at the time of my execution of this authorization, whichever occurs sooner. I agree and direct that this assignment, authorization and direction shall be automatically renewed, and shall be irrevocable for successive periods of one (1) year or for the period of each succeeding applicable collective bargaining agreement between the Employer and OPEIU Local 153, whichever shall be shorter, unless written notice is given by me to the Employer and written notice is given by me to OPEIU Local 153, not more than twenty (20) calendar days and not less than ten (10) calendar days prior to either the expiration of each period of one (1) year, or the termination date of the applicable collective bargaining agreement between the Employer and OPEIU Local 153, whichever occurs sooner.

I voluntarily make this assignment, authorization and direction for check-off of dues, fees and assessments, pursuant to the provisions of Section 302(c) of the federal Labor Management Relations Act, as amended. I agree that this assignment, authorization and direction for check-off of dues, fees and assessments is not conditioned on my present or future membership in OPEIU Local 153, nor is it to be considered given in return for my union membership.

Dues, contributions, or gifts to Local 153, Office and Professional Employees International Union, AFL-CIO are not tax deductible charitable contributions for federal income tax purposes. Dues paid to Local 153, Office and Professional Employees International Union, AFL-CIO, however, may qualify as business expenses and may be deductible in limited circumstances subject to various restrictions imposed by the Internal Revenue Code.
