 DIY Junk Removal – Lift Gate Vehicle Use Agreement & Liability Waiver

 

🔧Equipment Use & Training

CRS-Clutter Relief Services provides a 10–15 minute on-site training session to ensure the customer understands and is comfortable using the lift gate. This includes:

• Demonstration of safe lift gate operation
• Review of safety precautions and emergency stop procedures
• Confirmation of customer understanding and comfort level

 

🚫 Vehicle Use Restrictions

The vehicle provided is for on-site use only. The customer agrees to the following:

• The vehicle will not be driven off-site under any circumstances
• The vehicle may be started only to operate the lift gate or to move the vehicle forward or backward to accommodate loading/unloading
• The vehicle will be operated only by the trained customer listed above

  

⚠️ Liability Waiver & Assumption of Risk
By signing below, the customer acknowledges and agrees:

• They have received adequate training and feel confident operating the lift gate
• They assume all risks associated with the use of the lift gate and vehicle
• CRS-Clutter Relief Services is not liable for any injury, damage, or loss resulting from use of the equipment, including but not limited to:
• Improper operation of the lift gate
• Personal injury during loading/unloading
• Property damage caused by vehicle movement or equipment use

This waiver applies to the customer and customer assumes liability for any individuals assisting them on site.

I, the undersigned, have read and understood the terms of this agreement. I acknowledge receipt of training, accept all risks, and agree to abide by the vehicle use restrictions
