CAMPER REGISTRATION

We are so excited to offer another year of Created for a Purpose (CFAP) at Church of Our Saviour!  

CFAP is a week-long creative arts camp that teaches rising 3rd-8th grade girls that they are a unique masterpiece created by God. Each day focuses on a different project station along with Biblical teaching emphasizing that God has made everyone with different talents and skills to be used for His glory and His purposes. 

This ministry allows girls to create, sing, be silly, and to learn about Jesus Christ in a way that impacts how they live their lives.  They walk away with tangible reminders of the God who has made them, loves them, and desires a relationship with them!

HERE ARE THE DETAILS

• Camp is held June 22nd-26th from 9am - 1:30pm
• The registration fee is $200 per camper. This fee includes all camp supplies, camp T-shirt, and lunches each day.
• A deposit of $100 is required to hold your child's spot.  Scholarships are available upon request.
• Our registration limit is 50 campers and a waitlist will be available once the limit is reached. 

In consideration of the benefits to be derived and in view of the fact that Church of Our Saviour is a religious organization, membership of which is voluntary, and having full confidence that every reasonable precaution will be taken to ensure my and/or my  child(ren)'s safety and well being during Created for a Purpose, Built for a Purpose, and/or Vacation Bible School I hereby agree to my and/or my child(ren)'s participation and waive all claims against the leaders, sponsors, staff, clergy, volunteers, and Church of Our Saviour.

I give my permission for my and/or my child(ren)’s photo to be used for Church of Our Saviour, Created for a Purpose, Built for a Purpose, and/or Vacation Bible School promotional purposes as deemed useful by the camp director and/or church staff.  Names of the children will not be released.

I acknowledge that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. I know that COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

I understand that Church of Our Saviour (the Church) cannot guarantee that I or the Participant will not become infected with COVID-19.  Further, I acknowledge that attending the Church could increase my risk and the Participant’s risk of contracting COVID-19.

By signing this Consent, I acknowledge the contagious nature of COVID-19, that COVID-19 can cause serious injury, illness, permanent disability, and death, and that that an inherent risk of exposure to COVID-19 exists in any place where people are present.  Further, I voluntarily assume the risk that the Participant and/or I may be exposed to and/or infected by COVID-19 by attending Created for a Purpose, Built for a Purpose, and/or Vacation Bible School, including but not limited to the Activities, that such exposure or infection may result in personal injury, illness, permanent disability, and death, and that such exposure and risk may result from the actions, omissions, or negligence of me, the Participant, and others, including, but not limited to, Church staff, volunteers, program participants, and their families (collectively, the “COVID-19 Risks”).

I voluntarily agree to assume all of the COVID-19 Risks and accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or the Participant may experience or incur in connection with the COVID-19 Risks.  For myself and on behalf of the Participant Parties, I hereby release, covenant not to sue, discharge, and hold harmless the Church Releasees of, for, from, and against all past, present, and future Claims related to COVID-19 and the COVID-19 Risks. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Church Releasees, whether a COVID-19 infection occurs before, during, or after participation in any Church events or programs, such as Created for a Purpose, Built for a Purpose, and/or Vacation Bible School
