Following are the terms of adoption:

1) I agree, to the best of my ability, to keep this bird in my possession. I understand that parrots and other exotic birds can live long lives with the proper care, and I’m prepared to make the commitment to care for this bird for the remainder of its/my life. I agree to make arrangements for this bird’s care in case the bird outlives me. *

2) I will not sell this bird under any circumstances, nor will I give the bird away. If, for any reason, I am no longer able to care for the bird, I will make arrangements to return the bird to the Parrot Outreach Society rescue team. *

3) I will not breed this bird. I understand that there are many birds out there in need of a good home. I will not encourage this bird to breed by placing a breeder box or such in its cage. If this bird or its new mate lays eggs, I will remove those eggs within a few days and replace them with fake eggs to allow the female to recoup from laying its clutch.*

4) I understand that this bird comes with no health guarantee. I understand that birds may be infected with or act as a carrier of visually undetectable diseases, and I will not hold the Parrot Outreach Society liable under any circumstances if the bird is found to be ill and/or if any illness negatively impacts any members or any other birds in my household*

5) If I am adopting a “senior” parrot I realize that it may have a short life span even if cared for properly. I understand this bird has been tested for Chlamydia (a zoonotic disease), PBFD, Polyoma and Pacheco’s virus ONLY, no other blood work has been done. We do not guarantee the bird was examined by a vet.*

6) I understand that this bird will require professional medical treatment and annual exams, and I am financially able and willing to provide for that care. I understand that all medical costs are my responsibility. If my circumstances change and I find I am not financially able to provide necessary veterinarian care for this bird, I will contact the Parrot Outreach Society’s rescue team it was adopted from, immediately.*

7) I understand that this bird should be examined by a licensed avian veterinarian immediately and kept in quarantine under a separate air system from any other birds in my household until the veterinarian can determine the bird is disease free. Quarantine periods should last at least 60 days unless your avian veterinarian recommends a different duration of time. *

8) I will notify the Parrot Outreach Society’s rescue team, it was adopted from, immediately if this bird dies within 90days. A necropsy will need to be performed to determine the cause of death because other rescue birds and/or other birds in your household may have been exposed to the illness that this bird succumbed to. *

9) I understand that this bird may have suffered neglect and/or abuse in its previous home before arriving to Parrot Outreach Society. As such, that it may require a great deal of training and socialization. I agree to work with the bird WITHOUT using any form of negative reinforcement, which could cause additional psychological or physical harm to the bird. I understand that the adjustment period will be required love, patience and consistency on my part. *

10) I understand that the Parrot Outreach Society assumes no responsibility for the disposition or behavior of the bird, nor any harm or damages which might result from adopting the bird. I understand that the bird could bite me or others, that the bird could be a screamer (constant loud vocalizations), or could exhibit other undesirable behaviors. I understand that it will take time to work though these problems with the bird and that there is no guarantee that the problems can be resolved. I agree to seek help, if necessary, to properly deal with any problem behaviors this bird might exhibit. *

11) I agree to provide this bird with proper housing in a safely constructed and properly sized cage (specific to its breed and size), furnished with an appropriate variety of perches and toys, clean water, and healthy pellet and fresh food. The cage will be kept clean and water dishes will be scrubbed with hot,soapy water at least once a day. A variety of food will be provided – pellets and seed (if appropriate for this species), along with fresh vegetables and other safe table foods. I will purchase several toys and rotate them weekly to keep the bird mentally stimulated.*

12) This bird will be kept as a companion bird and will not be put on public display nor be used in any fashion for monetary gain. I agree to provide this bird with daily attention, interaction, and affection. I understand that parrots are social creatures and need the company of a feathered flock and/or human.*

13) I agree to allow members of the Parrot Outreach Society’s rescue team to contact me and/or enter my home at any reasonable time to check on the bird after the adoption has taken place. There may be one or two home visits to ensure the bird is doing well in its new home. If it’s determined that the bird is not receiving proper care, I agree to return the bird to the Parrot Outreach Society’s rescue team immediately. *

14)This agreement shall be interpreted under the laws of the State of Florida. Any litigation under this agreement shall be resolved in the trial courts of Charlotte County, State of Florida.*

15) I agree that no claim or legal action will betaken against the Parrot Outreach Society, its officers and agents, by reason of this adoption or any action of this bird. I accept full legal responsibility for this bird and its actions*

16) I understand that this is a legal and binding contract. Failure to comply with any paragraphs or requirements in this contract will constitute a breach of contract for which I agree to return the bird to the Parrot Outreach Society and pay reasonable attorney’s fees and court costs if this matter is referred to an attorney for legal action. *

17) I hereby state that I have never been charged with nor convicted of animal neglect or abuse in Florida or in any other state, nor has anyone else in the household in which this bird will reside.*

18) I agree to keep this bird on a pellet diet and not feed seed. Every bird at Parrot Outreach has been switched to a pellet because it is the healthiest diet available, supplemented with fresh fruits and vegetables.*

19) In the event that I do not comply with the terms of this Agreement, or the bird is abused, neglected, bred, or sold; Parrot Outreach Society holds the right to recover the bird from me (the adopting party) upon demand, and the bird will be surrendered to Parrot Outreach Society immediately. In addition, without waiving any of Parrot Outreach Society's rights under this Agreement, if I do not surrender the bird to Parrot Outreach Society, I will owe Parrot Outreach Society a payment in the amount of $5,000 agreed between Parrot Outreach Society and I as just and reasonable liquidated damages to compensate Parrot Outreach Society for costs and expenses in connection with my breach of this Agreement and Parrot Outreach Society reserve their rights to reacquire the bird. In addition to liquidated damages, I (the adopting party) shall pay all legal costs and expenses, including expert witness fees, depositions, attorney fees and any other legal fees incurred by Parrot Outreach Society in resolving any dispute with retrieving said bird*

20)I give Parrot Outreach Society Consent to do a background check ONLY to ensure no animal cruelty or animal related charges have ever been brought against me*

21) This Agreement is the entire agreement between Parrot Outreach Society and I. This agreement supersedes any prior understandings between Parrot Outreach Society and I (the adopting party) with respect to the subject matter of this agreement. No modification of this Agreement will be valid unless inwriting duly signed by both Parrot Outreach Society and I (the adopting party). This Agreement is binding upon me (the adopting party) and my heirs, assigns, successors, personal representatives and executors involved with said bird during the bird's lifetime.*

The Parrot Outreach Society is a not-for-profit group. We gladly accept and appreciate all donations (monetary and supplies), but please be aware that donations are non-refundable.

I have read all pages of this contract, understand it, and agree to each and every one of its terms.
