  • Roles with Plant City Homeschool Collective may involve direct contact with minors. As a result, selection for employment or engagement in any paid or authority-based role is contingent upon the successful completion of a Florida Level 2 background screening, including fingerprinting, in accordance with applicable state law.

Florida Level 2 screening includes state and national criminal history checks and is required for individuals working with or supervising children pursuant to Florida Statutes §§ 435.04, 402.305, and 1012.32, among others.

By submitting this application, I authorize Plant City Homeschool Collective to obtain and review the results of a Florida Level 2 background screening for the purpose of determining my eligibility for employment or engagement. I understand that screening results will be reviewed in accordance with applicable federal and state laws, including Florida Statutes Chapter 435, and that a prior record does not automatically disqualify an applicant unless it includes a disqualifying offense under law.

I understand that I may not begin or continue in any role requiring screening until the background screening process is completed and reviewed by PCHC administration. I also understand that providing false, incomplete, or misleading information at any stage of the application or screening process may result in disqualification or termination.

If selected to move forward, Plant City Homeschool Collective will provide instructions for required screening and will cover applicable screening costs.

This obligation applies regardless of role title or employment status when working with or around children.

Failure to report suspected abuse, abandonment, or neglect may result in criminal penalties under Florida law.

  • Application Attestation

I certify that the information provided in this application is true, accurate, and complete to the best of my knowledge.

I understand that submission of this application does not guarantee an interview, offer, or employment.

I understand that any misrepresentation, omission, or false statement may result in disqualification from consideration or termination of employment or engagement.

I understand that, if selected for a role, my employment or engagement with Plant City Homeschool Collective will be at-will, meaning that either I or Plant City Homeschool Collective may end the relationship at any time, with or without cause or notice, in accordance with applicable Florida law. This at-will relationship applies regardless of role, title, or length of service.
