I, the parent/guardian grant consent for my child to take part in individual and/or group psychology sessions facilitated by Peak Performance Psychology Limited. These sessions aim to provide psychological support and guidance to enhance my child's performance and well-being in various aspects of sport and life.
Adam Bracey acts as a supervisor to the Consultant. Therefore, content discussed in sessions may be discussed with the supervisor. He is a HCPC Registered Sport Psychologist and BPS Chartered Psychologist. If you have any concerns, contact by email: (adambraceyspsych@gmail.com).
Confidentiality:
I understand that the information shared by my child during the sessions will be treated as confidential and will only be disclosed to appropriate individuals if there is a concern about my child's safety or the safety of others, or if required by law.
Parent/Guardian Involvement:
I understand that my involvement and support is important to the success of my child’s sessions, and I commit to staying up to date regarding my child's progress.

I understand that these sessions are not a substitute for medical treatment or therapy, and I release Peak Performance Psychology Limited from any liability that may arise as a result of my child's participation in these sessions.
Payment Information:
Please note that session payments are taken at ~11am GMT on Monday mornings (when you have a session booked that week).
If payment fails we will try again at ~midday GMT Monday.
If payment fails again we will try again at ~11am GMT Tuesday.
If payment still fails we will pause sessions and charge a £5 fee to restart. (This is super rare!).
