Tall Tails Rescue and its representatives reserve the right to deny or refuse adoption to any person for any reason at their discrepancy.

By applying for the dog listed at the top of this form you agree to the following terms should you be selected to adopt:

I / we agree to take this dog to the vet within 30 days of adoption for a check up.
I / we understand that this is a rescued animal and although we do our best to determine the temperment and current health status of the animal there is no guarentee on health or temperment.
I / we agree that if we choose to surrender this dog back to Tall Tails Rescue, we will foster the dog until a home can be found.
I / we agree to continue heartworm and flea prevention for the duration of this dogs life.
I / we agree to return this dog to the adoption liaison point of contact Alicia Ruiz (360-485-9136) if for any reason I / we no longer wish to keep this dog.
I / we understand that re-homing or surrendering this dog to a shelter is a breech of a legal contract and I / we may be fined and/or prosecuted at the fullest extent of the law if I / we choose to do so without obtaining written permission from Tall Tails Rescue or Alicia Ruiz.
