LIABILITY WAIVER
I affirm that the participant ("Child") has my consent to participate in any camp or program offered by PacificPine Sports Limited and/or PacificPine Tennis Limited. I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child or I may sustain as a result of said participation. I understand that there is an element of risk inherent in youth sports, and as parent/legal guardian of the Child I take full responsibility for his/her actions and physical condition. Furthermore, I understand that certain risks, dangers, and injuries due to things such as inclement weather, slipping, falling, exposure to infectious diseases, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to youth sports programs exist. Therefore, it is recognized that it is impossible for PacificPine Sports Limited and/or PacificPine Tennis Limited to guarantee absolute safety. I release and agree to indemnify and hold harmless PacificPine Sports Limited and/or PacificPine Tennis Limited and its representatives, volunteers, and agents from all claims or liability for loss, damages and/or injuries, to the named Child and/or any third parties person or property, caused by and resulting from any causes whatsoever.
MEDICAL RELEASE
I hereby give full permission for any and all medical treatment necessary to be administered to the Child in the event of an accident, injury, sickness, etc., under the direction of PacificPine Sports Limited and/or PacificPine Tennis Limited until such time as I may be contacted. I further give permission to the PacificPine Sports Limited and/or PacificPine Tennis Limited staff to use and disclose the medical information provided on this form for purposes related to such medical treatment. The release is effective for the time during which my child is participating in any of the camps or programs held at the ATP Official Development Center-Waterfall. I also hereby assume the responsibility for payment of such treatment.
REMARKS:
- No refund under any circumstances.
- Make-up sessions will be offered in the event of cancellations due to weather conditions at our facility or coaches sickness. These sessions will be rescheduled to the last 2 weeks of the term only, based on coaches availability, and a maximum of three (3) sessions can be reedemed per child, per term. Lessons cannot be carried to the upcoming terms. If the options provided for rescheduling of the class will not be convenient, the lesson will be forfeited and charged.
- Each child is entitled to a maximum of two (2) make-up sessions per term due to sickness. Doctor's note will be required each time within 48hs. These sessions are to be carried during the term and cannot be made up after the term ends, or carried to the next term. If the options provided for rescheduling of the class will not be convenient, the lesson will be forfeited and charged.
- Sessions missed due to absence or travel do not qualify for make-up sessions and will be charged.
- In the events of a long term injury or illness that will prevent the student from attending multiple lessons, each case will be reviewed independently (with medical note provided) to decide on possibility of make-up sessions.
- In case of rain or Typhoon 3 signal or higher is hoisted, all lessons will be postponed (Refer to our severe weather arrangements).
-If a session is canceled due to rain within 30 minutes, it will be considered a rainout and will qualify for a make-up session. If the session is interrupted by rain after 30 minutes, it will be considered a full lesson and charged accordingly.

- ATP Official Development Center - Waterfall reserves the right of final interpretation.
-A minimum of 3 kids is needed to start a group. In the event of not having enough signups to start the class, an alternative time and day will be provided.
PHOTOGRAPHY OR VIDEO TAKING
I acknowledge that photos and videos may be taken during the event for marketing activities and social media.
