I have, following consultation, consideration and discussion, agreed to go along this new PNS approach. I am fully aware that it is of a holistic nature and does not serve as a substitute for professional medical advice, examination, diagnosis or treatment. I have had the process explained to me and I fully understand that it may take several sessions before I notice any long-term benefits. This will depend not only on my lifestyle, the medication I might be on, and my general health but also my commitment to use and practice the methods taught.

I acknowledge having given my personal details for the PNS provider’s use. I understand that my information is protected and I give my consent to store and use these details. I understand that having been untruthful with my details or having failed to give enough relevant information could affect the result of the approach.

The PNS consultant does not make any diagnose nor claim to cure any medical condition in the same regard as a physician, as her opinion is that of a holistic, complementary and alternative intervention and her professional opinions, advice, examinations, and recommendations do NOT constitute the medical advice of a doctor/physician.

I will seek the advice of my doctor or other qualified health providers in the case of a medical emergency, medical ailment or in respect of general health questions without delay.
