Fees, Policies, and Agreements

Fees:

In-Person Treatment:
- Initial Session $240 (90 minutes)
- Follow-Up Sessions $160 (60 minutes)

Remote Treatment via Video Conference or Phone: $164 (60 minutes)

Cancellation Policy:

A minimum of 24 hours notice is required to cancel any and all appointments. Failure to do so will result in an automatic $150 cancellation fee. Treatments cannot resume until all payments have been made.

Policies and Agreements:

I understand and agree that I am ultimately responsible for the payment of all professional services rendered.

I understand that I am obligated to give a minimum of 24 hours notice to cancel or reschedule all appointments. In the event that I fail to give 24 hours notice, I agree to pay a $150 cancellation fee.
