Adoption/Foster Application

This questionnaire is designed to help the shelter and yourself determine if a specific pet is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. This shelter will select and approve the best fit for all animals in our care. This decision is based on many factors, such as current living situation, family members, other pets in the home, ages of children, etc. We reserve the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions. We do NOT discriminate based on race, sex, or religious beliefs.

By completing this form, all parties hereby ackowledge that the animals's best interest comes first and foremost. And we do not approve based on order of receipt of the application or any other factors except what is best for the animal in their situation.

We are located in Northern West Virginia, New Martinsville. We are a non- profit 501C3, funded by donations and we try to help as many homeless and unwanted pets as we can in our area. All of our pets are currently fostered by our small staff and receive complete medical care, which includes worming, vaccinations, spaying/neutering prior to their adoption. (We can offer micro-chipping on request)
