CANCELLATIONS
There is a cancellation fee of 20% the cost of service for any cancellation with less than 24 hours notice.
No Call/No Text/No Show:
No shows will be charged a no show fee in the amount of 100% the total cost of service.
TARDINESS
Late Arrival: Your appointment will be automatically cancelled after 15 minutes of no call no text or no show.
In order to be able to receive the full Piscean Touch experience, the full allotted time given for the appointment will need to be utilized. Understand that arriving late will result in less time being allowed for services due to your tardiness. No extra time will be added.
However, if Piscean Touch is at fault for not being able to start within 15 minutes of your appointment time, a credit or extra time will be applied towards that appointment.
REFUNDS
All booking fees and prepayments are non refundable and non transferrable. There are absolutely no refunds on any services. If you experience an allergy or any discomfort and are unable to continue wearing lash extensions, they will be removed free of charge.
Credit Card Authorization
I authorize Piscean Touch to keep my credit card on file until I request a cancellation request that it be terminated via an emailed request.
Covid Liability:
Symptoms of COVID-19 include but are not limited to:
•Fever
•Fatigue
•Dry Cough
•Difficulty Breathing
1) I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above WITHIN THE LAST 14 DAYS.
2) I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.
3) I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.
4) I affirm that I, as well as all household members, have nottraveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE PAST 30 DAYS.
5) I understand that Piscean Touch, LLC cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.
Piscean Touch, LLC are following these enhanced procedures to prevent the spread of COVID-19:
•Prepayment for services available if clients prefer contactless checkout
•Additional time included between appointments to prevent client contact with each other
•Guests accompanying those scheduled for services discouraged
•Each client is required to have on a mask upon entrance, wash hands upon arrival and before leaving
•Your artist will thoroughly clean hands in between each client
•All products used during treatment will be disinfected and disposable items will be used when possible
•All surfaces will be wiped thoroughly with hospital grade disinfectant before and after each client according to the manufacturer's directions
By signing below, I agree to each statement above and release Piscean Touch, LLC from any and all liability for the unintentional exposure or harm due to COVID-19.
Piscean Touch, LLC agrees to abide by these standards and affirms the same.
Appointment Instructions
On the day of your appointment you agree to...
-No coffee or heavy fluid consumption prior to appointment.
This will cause frequent bathroom breaks.
-Use the bathroom BEFORE your appointment time.
You should be seated on the recliner at your appointment time so that I may prepare you for service.
-Silence and put away phones.
-Come in with no eye makeup or foundation.
-Not to bring any extra guests/children.
This is your time to clear your mind and relax. Having to entertain extra guests is distracting and slows down servicing.
-Communicate freely.
I will ask periodically if you are comfortable. Never hesitate to ask questions, make a music change request or ask for a bathroom break if needed.
Piscean Touch has the right to refuse service and you may be banned from scheduling future bookings by not respecting the above policies.
