I understand this is a 2-day Basketball Agency Exposure Camp hosted by Rep 3Sixty.

Event Details:
Saturday, April 25: 10:30 AM – 2:30 PM
Sunday, April 26: 10:30 AM – 3:30 PM

Location: MSK Marlboro (2 Timber Lane, Marlboro, NJ 07746)

Each player is guaranteed 3 full games, along with skill work and evaluations.

I understand that participation includes:

• Game film from all games
• Full stats from games
• A detailed scouting report from an NBA/FIBA Agent at Rep 3Sixty
• Official player ranking from the camp
• Post EVENT ONLY Exposure and marketing to college and professional coaches and scouts. 

I understand this camp is limited to 30 boys players for grades 9-12 ONLY.

*I Understand this is NOT an NCAA certified event. There will NOT be college coaches in attendance at the event. This is strictly for our Agency Exposure and marketing for your athlete*

THIS EVENT IS A PRIVATE AGENCY EVENT FOR EVALUATION!

SECTION 5: LIABILITY WAIVER, ASSUMPTION OF RISK & MEDIA RELEASE

"I, the undersigned parent or legal guardian of the participant listed in this registration, acknowledge and agree to the following:

ASSUMPTION OF RISK
I understand that participation in basketball activities, including training, drills, scrimmages, and organized games, involves inherent risks of injury, including but not limited to serious bodily injury, permanent disability, or death. I voluntarily assume all risks associated with participation in the Rep 3Sixty College Exposure Camp.
RELEASE OF LIABILITY
On behalf of myself and the participant, I hereby release, waive, discharge, and hold harmless Rep 3Sixty, its owners, agents, employees, contractors, coaches, referees, facility operators (including MSK Marlboro), and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or related to any injury, illness, damage, or loss sustained during participation in the camp, whether caused by negligence or otherwise.
MEDICAL AUTHORIZATION
In the event of an emergency, I authorize Rep 3Sixty and its staff to obtain medical treatment for the participant. I understand that I am responsible for any and all medical expenses incurred.
PARTICIPANT RESPONSIBILITY
I acknowledge that the participant is physically fit and able to participate in athletic activities. I agree that the participant will follow all rules, instructions, and safety guidelines provided by camp staff. Failure to do so may result in removal from the event without refund.
MEDIA RELEASE & PUBLICITY RIGHTS
I grant Rep 3Sixty the irrevocable right and permission to film, photograph, record, and use the participant’s name, image, likeness, voice, and performance for marketing, promotional, recruiting, and commercial purposes, including but not limited to social media, advertisements, scouting materials, and distribution to college and professional coaches and scouts, without compensation.
NO GUARANTEE OF OUTCOMES
I understand that participation in the camp does not guarantee athletic advancement, scholarships, contracts, or recruitment opportunities. Evaluations, rankings, and exposure are subjective and based on performance.
REFUND POLICY
I understand and agree that all registrations are final and non-refundable unless otherwise stated by Rep 3Sixty.
GOVERNING LAW
This agreement shall be governed by and interpreted in accordance with the laws of the State of New Jersey.
