- Completed Application Form
- Necessary documents to confirm your identity
- Passport / ID Card
- Birth / Marriage Certificate
- Utility Bills / Bank Statement
- Details of 3 referees - current and last employers including e-mail addresses
- Evidence of Right to Work in the UK
- Original / Certified Registration certificates, professional qualifications, membership of professional bodies
- NMC Original Statement of Entry
- NMC Annual Statement of Entry
- Police check from original country of origin - if you have been in the UK less than six months
- Your last appraisal from your employer
- Original IELTS Certificate - if applicable
