Add Form Prefill Info

This form will collect some information from the user that can be used to prefill other forms. Current forms supported:
- Test Prefill and Photos
- Payment Request - version 4 and later
Some manual processing needs to be done after this form is submitted.
1. Go to Tables (Submissions) and download "all" of the entries into an Excel file.
2. For each form, refresh the "Spreadsheet to Form" widget which is used for the Unique Code field. Upload this Excel file. Note: Don't forget to select Unique Code for the "Access code column.
Purpose

This form can be used to provide some common information that you might use in filling out other RCC forms, such as name, email, and phone. By supplying a Unique Code on the other forms, the other fields can be prefilled.

Currently it will be used for the Payment Request form but may be added to other forms in the future.
Please provide a short unique code:
- It will be the code that you type into another form to prefill the other fields.
- Some possible examples:
  - Your 3 initials.
  - A short word - This would be useful if you want to use voice input on your phone.
  - A meaningful number, such as the your birth year, house number, etc.
- The code that you provide will be case sensitive when you use it in a form.
- If the code is already used by someone else, you will be notified

  The Name field uses the Calculation widget to reformat the Full Name field into 1 string. Otherwise, it is saved in a format that doesn't work correctly with the Spreadsheet to Form widget that is used to prefill other forms.
