Section 1 — Individual Attributes (maximum of 60 points)
The following are individual attributes that reflect the qualities necessary to receive this award. Please provide a brief, descriptive example for each demonstrating expertise and experience of the applicant. Activity at the chapter, regional, and/or RIMS (Society) level that benefits the chapter should be included and will be considered. Activity can also be with more than one chapter for the candidate's entire career.
A. Promoting the health of RIMS chapter(s). How does the candidate promote risk management and RIMS at the chapter level? *
B. Positive Public Image. What are examples of how the applicant helps to ensure the positive reputation of RIMS. *
C. Team Player. Provide specific examples that demonstrate how the candidate is a team player who works well with other Chapter members.*
D. Leadership. Please identify all significant RIMS positions held by the candidate, including but not limited to Chapter or Society officer, director, committee chair, committee member, etc. Identify the position, timeframe and significant accomplishments for each position held. *
E. Creativity. Provide specific examples of how the candidate addressed a challenging issue. *
F. Advancing the risk management profession. Let us know what the applicant is doing to encourage growth of our profession through mentorship, legislative involvement, continuing education, presentations, etc.*

Section 3 - Letters of Recommendation (maximum of 20 points):
Two electronic letters of recommendation from risk management professionals are required and must be from two different sources (one of which must be from a risk professional, but the Nominator is specifically excluded). Recommendations may come from colleagues, chapter board members, educators, students, or members of the RIMS Board of Directors. Members of the Member and Chapter Services Committee may not provide letters of recommendation. If recommendation letters are not submitted, the application will not be eligible for consideration.
