Adoption Application

Please fill out this application for Rebel for Life so we may assist you in finding a compatible companion to join your family. Please understand that not all families and pets are suitable. We want to ensure not only the welfare of the animals by placing these pets in the best possible home, but also for the families adopting them to be successful together.

WE RESERVE THE RIGHT TO DECLINE ANY APPLICATION FOR ANY REASON.

The adoption fee includes the cost of spaying/neutering, heartworm testing if over 6 months of age, age appropriate vaccinations, deworming, monthly preventatives, and microchip application. It rarely, if ever covers all expenses, so therefore additional tax deductible donations are welcome.

YOU MUST BE AT LEAST 25 YEARS OF AGE TO ADOPT.

Most puppies and many of our adults will NOT have been housetrained and will require your time to learn this appropriately. 

Understand that puppies go through a chewing phase, and during this time it is very important to have plenty of toys on hand and use them to redirect this behavior. Keep personal belongings, valuables, and electrical cords hidden.

We recommend getting a kennel to keep your newly adopted pet in at night or during the day when you are not home until they are comfortable in their new space.

 Always go slow when introducing current pets to newly adopted pets as this can be a stressful transition for the adopted pet being in a new environment. 

Also, if you are a first time pet owner we recommend that you take your new pet to obedience taining courses and get them in a doggy daycare program to keep them well socialized.
