    After completing this form please upload copies of the following documents listed below. Please note that there is a $30 processing fee to submit this application. 


    Individual Applicant(s) and Guarantors:

    • Individual Federal tax returns for yourself and spouse (if applicable) for past two (2) years
    • Government/state issued photo ID’s (driver’s license,passport, green card)
    • Bank statements for all accounts for the past three (3) months
    • Most recent quarterly Stocks, Bonds, Mutual Fund Statements, including 401K accounts
    • Professional resume(s) and /or work experience
    • If this is a relocation, sales report for current business for last 12 months
    • Business plan
    • Any Bank Pre-approval letters if taking out a personal or business loan
    • List of additional businesses currently involved with, including any franchise arrangements

    If an Applicant is MARRIED, BOTH SPOUSES must fill out the application, BOTH SPOUSES will be screened as a Tenant, BOTH SPOUSES will be listed as a Tenant on the Lease and BOTH SPOUSES will be required to sign the Lease.


    Corporate Applicant:

    • If audited statements are not available, Business Federal tax returns (all schedules) for prior two (2) years and most recent YTD
    • Company bank statements for past three (3) months
    • Guarantor’s Federal tax returns (all schedules) for prior two (2) years
    • Guarantor's bank statements for past three (3) months
    • Any Bank Pre-approval letters if taking out a business loan
    • Photo ID of authorized signor of entity, as well as all individual(s) personally guaranteeing the Lease

    ** Please note: Most Leases held by a corporate entity require a personal guarantee and will be determined upon the review and full evaluation of a complete application. Please complete the guarantor section of the application. If a Guarantor is MARRIED, BOTH SPOUSES must fill out the Guarantor section and BOTH SPOUSES will be required to sign a Personal Guarantee.

    Landlord reserves the right to request additional documentation as deemed necessary. In the event information is missing or incomplete, the processing of the application may be delayed or denied. Accordingly we urge you to carefully read the instructions and follow them thoroughly.
