By clicking "Submit Registration" below, I, the undersigned parent or legal guardian, acknowledge that I have read, understand, and voluntarily agree to all of the following terms and conditions on behalf of myself and the minor athlete named in this registration form:
1. ASSUMPTION OF RISK I acknowledge that participation in running, cross country training, track & field, and related athletic activities involves inherent risks of injury, illness, death, and loss or damage to personal property. These risks include, but are not limited to, physical contact, overexertion, adverse weather conditions, equipment failure, and the actions of other participants. I voluntarily assume all such risks on behalf of myself and my minor child, whether known or unknown, foreseeable or unforeseeable.
2. RELEASE AND WAIVER OF LIABILITY In consideration of my child's participation in the Roeder Multisport Summer XC Training Program and any related activities, competitions, or events, I, on behalf of myself, my child, and our heirs, executors, administrators, and assigns, hereby irrevocably release, waive, discharge, and covenant not to sue the following parties (collectively, "Released Parties"):
Roeder Multisport LLC, its owners, employees, coaches, contractors, and volunteers
USA Track & Field (USATF), its officers, officials, employees, and volunteers
The Indiana Association of Track & Cross Country Coaches (IATCCC) and Indiana Youth Track Association, their officers, officials, employees, and volunteers
YES Athletics and any affiliated organizations
Any track club, meet host, or facility owner/lessee at which my child participates
All owners, lessees, and operators of facilities used in connection with the program, and their respective officers, employees, agents, and volunteers
I release all of the above from any and all liability, claims, demands, losses, damages, costs, and expenses — including court costs, settlement costs, and reasonable attorney fees — arising out of or related to my child's participation, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by Indiana law.
3. INDEMNIFICATION I agree to indemnify, defend, and hold harmless all Released Parties from and against any claims, damages, losses, or expenses, including attorney fees, arising out of or resulting from my child's participation in the program or any related activities.
4. EMERGENCY MEDICAL AUTHORIZATION I authorize the coaches, staff, and volunteers of Roeder Multisport LLC to consent to and authorize emergency medical treatment for my minor child if I cannot be reached in a timely manner. I acknowledge that Roeder Multisport LLC assumes no financial responsibility for medical costs incurred.
5. MEDIA RELEASE & INTELLECTUAL PROPERTY I grant Roeder Multisport LLC to photograph, film, record, and otherwise capture my child's name, likeness, image, voice, video, and athletic statistics, and to use and display such content in any media format — including but not limited to social media, websites, print, and promotional materials — without further consent or compensation.
6. CODE OF CONDUCT I agree that my child and I will conduct ourselves in a respectful and sportsmanlike manner at all times. Roeder Multisport LLC reserves the right to dismiss any athlete from the program for misconduct without refund.
7. REFUND POLICY I understand that all registration fees are non-refundable once payment has been submitted, except at the sole discretion of Roeder Multisport LLC.
8. ACKNOWLEDGMENT I affirm that I am the parent or legal guardian of the minor athlete named in this registration, that I have the legal authority to enter into this agreement, that I have read this agreement in its entirety, and that I agree to its terms freely and voluntarily.
