  • While your contact information is not designated as a required form field, to properly process and respond to your complaint, it is important to be able to contact for additional details if necessary, or proposed resolutions if available.

  • Complaint Information

  • To properly respond to your complaint, the SFSS need details about the issue you faced. As a guide, make sure your description provides answers to the follwing questions:

    • What happened?
    • Where did this event take place (i.e. were you being served online, in an office, on the phone, via email,..)?
    • If you were dealing with a person, what was that person's name?
    • When did this event take place?
    • Where did the event take place?
    • How did you experience the event?
  • Should be Empty:
