In consideration of the acceptance of this entry by AAC and Sleeping Giant Dog Sports (the “Organizing Club”) and the opportunity to have the Dog participate, compete in and/or be judged at this event or trial (the “Event”), I, the undersigned, agree as follows:
1. I certify that I am the actual owner of the Dog, or that I am the duly authorized agent of the actual owner of the Dog and that the Dog is not a
hazard to persons, property or other dogs.
2. I agree that AAC or the Organizing Club for the Event may refuse my entry for any reason that it deems to be sufficient.
3. I understand and acknowledge that participating in the Event carries certain risks, including, but not limited to, injuries (including serious injuries
and death) to myself, the Dog, another dog under my control or persons attending as my guests (my “Related Parties”), damage to property and
other losses. I waive any and all claims that I have or may have in the future against, and release from all liability and agree not to sue AAC, the
Organizing Club or any of the directors, executives, committees, representatives, employees, volunteers or agents of either AAC or the Organizing
Club (the “Personnel”) for any injury, death, property damage or other loss that I or my Related Parties sustain due to any cause whatsoever,
including negligence, breach of contract, breach of duty, statutory or otherwise, or mistakes or errors of judgement of any kind.
4. I assume full responsibility for the conduct of myself and my Related Parties during the entirety of the Event. I agree to indemnify and save
harmless AAC, the Organizing Club and the Personnel from and against any and all claims that may be made against them or costs, fees, expenses
or liability (including, without limitation, lawyers’ fees on a solicitor and own client basis) incurred by AAC, the Organizing Club or the Personnel that
may arise due to:
(a) Any act or omission of myself, my Related Parties or any party for whom I am liable at law; or
(b) Aggressive behaviour by the Dog or any other dog under my control.
5. I have read, and do understand the rules of competition and the additional rules (if any) appearing in the trial schedule for the Event, as well as the
policies of AAC pertaining to discipline (collectively, the “Rules”). I agree to abide by the Rules and to conduct myself and the Dog in accordance
with the Rules. I understand that myself and/or the Dog may be subject to disciplinary procedures or sanctions for breach of the Rules and agree to
respect and be bound by any disciplinary decision made by AAC.
6. I understand that AAC and/or the Organizing Club may, in the course of administering the Event or conducting its activities generally, collect, use or
disclose my personal information and that any personal information collected, used or disclosed by AAC and/or the Organizing Club will be treated in
accordance with AAC policy and applicable privacy and personal information laws. I hereby consent to the collection, use and disclosure by AAC
and/or the Organizing Club of my personal information.

Media Release & Rights of Privacy Waiver

I hereby grant to AAC and its affiliated companies, representatives, agents and assigns, all rights and permission to use or appropriate his/her and
their dog’s name, biography, likeness, photograph, voice, performing persona, or other indicia of identity for broadcast, telecast, cablecast, transmission or distribution in any format or media known now or in the future.

Waiver of Rights to Privacy, Publicity & Personality

Further, I hereby release Agility Association of Canada Inc., and its affiliates, representatives, agents and assigns from any claim or cause of action for invasion of the rights of privacy, right of publicity, right of personality, or any similar rights.

7. I have been given the opportunity to read and understand the terms of this Agreement before signing it, and have in fact done so. I understand
that by signing this Agreement, I am restricting my legal rights and I sign this Agreement voluntarily and of my own free will.
The Personal Information Protection and Electronic Documents Act (PIPEDA) requires that consent be obtained for the collection and use of personal
information. The information collected by Sleeping Giant Dog Sports will be used for: registering and listing competitors in agility events, submitting results, as
required by the sanctioning body, and contacting competitors by email, mail or phone for future agility events such as trials and seminars. The information will
be retained by Sleeping Giant Dog Sports for a period of two (2) years and disposed of in a manner that ensures information protection.

Parent/Guardian must sign if handler is under 18 years of age.

PLEASE READ THE DESCRIPTIONS CAREFULLY AND SELECT THE CLASS AND JUMP HEIGHT THAT CORRELATES TO YOUR DOGS MEASURED HEIGHT*:

REGULAR CATEGORY:
Dogs will jump their full jump height as determined by their measurement in the table above. The course may include spread jumps and will include a full height A-frame.

SPECIALS CATEGORY:
Dogs will run one height (4") lower than the height indicated by their measurement. Any dog eligible for entry may run in the special category. There will be no spread jumps used and the A-frame will be lowered to 5'. Once a dog has entered an official trial in the Specials class, they may no longer compete in the Regular class unless they make a special application entitled “Request for Official Move from Specials Class Level to Regular Class Level”. This move may only be done once during a dog’s career.

VETERANS CATETORY:
This Category is for dogs seven years of age or older. This class is also open to dogs five years of age or older who have been competing in the Specials class level for a minimum of twelve months.

All Veteran Category dogs must jump at least one jump height lower than the lowest of the two Regular jump height options (Single Drop Veteran Dog), and may jump two jump heights lower (when a lower jump height is available), at the handler’s discretion (Double Drop Veteran Dog). Handlers can opt to have their dogs jump at either allowable height at any given trial, provided they enter and jump the same height for all classes at that particular trial.

Accessibility Option: Senior Handlers 65 years of age and over, and handlers of any age with a disability may request a modified course time and a modified points requirement. Handlers can choose this option at any trial provided they use the Accessibility Option for the whole of the trial. The handler’s dog shall be entered at its current competition jump heigh and category, including Junior Handler. The modified time and reduced points requirements are a concession for the handler, not the dog. Proof of disability or age will not be requested by the host club. A handler must indicate on the entry form (i.e. by checking the AO box) if they are requesting an Accessibility Option in order to receive the modifedtime/reduced number of points.

$10.00 FEO RUNS! FEO is a great way to experience a new venue, introduce a novice dog, or reinforce your dog with a toy in the ring. Dogs entered as FEO at time of closing are eligible for this $10.00 reduced rate. FEO dogs may jump any height.

Handlers who enter as a regular run may choose to run FEO on the day of trial but for their regular entry fee.

Open to all dogs, 18 months of age or older with an AAC ID number, as described within the current AAC rules. The handler can choose to enter a dog FEO in any division, any class and/or any height but they will not be competing for placements, titles or prizes and the results will not be reflected on the dog’s permanent record.

• NEW! Food in an FEO RUN – The use of sealed food containers in the ring during an FEO run will be allowed. Food containers must be made of such material, structure, and in such away as to ensure there is no leakage of crumbs or treats and no change of accidental spillage should the container be tossed, dropped or grabbed by the dog. The handler cannot feed the dog during the run or in the ring.
  
  
• Toys will be allowed during FEO runs. Toys may be of any general configuration & must be in a good state of repair (no stuffing or fur coming loose). Handlers are permitted to interact with their dog by tugging the toy.
