Wisconsin & Racine Parental Choice Program (WPCP & RPCP)

Thank you for applying to the Wisconsin Parental Choice Program (WPCP) or Racine Parental Choice Program (RPCP) for St. Joseph Catholic Academy.  In order for your application to be verified, please submit the appropriate documentation by the 20th of each month that you are applying for.

New student documentation required:

• Proof of Residency
• Proof of Income

Returning student documentation required:

• Proof of Residency
  • Alternate Residency Form (ONLY to be used when the pupil is not living with his or her parent or legal guardian or when the parent or legal guardian does not have any of the allowed documentation showing their residency at the address on the application.)

For students that already attend St. Joseph Catholic Academy- you may skip this step

If you are applying for a student that does not currently attend St. Joseph Catholic Academy, please review our Admissions Procedures and Policies Manual.  Once you have reviewed the manual, please complete the following steps:

• Sign this form acknowledging that you have reviewed and agree to the Admissions Procedures and Policies Manual.
• Complete a New Student Application for St. Joseph Catholic Academy.  Please be sure to upload a copy of the student's most recent report card- if applicable.

For cases where:

• The Parent's first or last name has legally changed; and
• The name provided in the WPCP or RPCP application is the correct legal name; and
• The name listed on the WPCP or RPCP application does not match the supporting residency documentation

The parent must provide additional documentation to verify the correct legal name (i.e. birth certificate, marriage certificate, divorce decree, certificate of naturalization, certificate of citizenship, etc).

 Please upload any supporting document that verifies the correct legal name if the name on the proof of residency does not match the name listed on the application.

 

EX:  My name is Jane Smith.  I applied for the WPCP program with the name Jane Smith.  My WE Energies bill that I am using as a proof of residency has by maiden name of Jane Jones.  I must provide a legal document verifying my name change from Jane Jones to Jane Smith.
