Please take 5-10 minutes to complete the following 3 sections of this Recognition of prior learning form:

1. Personal Details – Enter your name and contact details.
2. IF your Surfguard record does not show a current HLTAID010 (Provide Basic Emergency Life Support) or HLTAID011 (Provide First Aid), please attach a copy of a current Statement of Attainment. Then sign the declaration. 
3. Upgrade Evidence – Confirm that you have met the evidence requirements.

Please note the following is required to submit this form:

• Knowledge of your SLS Hub Awards List
• A current HLTAID010 or HLTAID011 listed in SurfGuard
• Hold and be proficient in the SLSA Bronze Medallion
• Hold a PUA21010 or PUA21012 Certificate II in Public Safety (Aquatic Rescue) that aligns with the SLSA Bronze Medallion
