Please note the following is required to submit this form:

Knowledge of your SLS Members Area Awards List, e.g., expiry dates.

A current HLTAID010 or HLTAID002 Provide basic emergency life support listed in SurfGuard.

Hold and be proficient in the SLSA Bronze Medallion.

Hold a PUA21010 Certificate II in Public Safety (Aquatic Rescue) that aligns with the pre-2020 SLSA Bronze Medallion.
