** YOU MUST HAVE YOUR HONOREE'S PICTURE, SERVICE BRANCH AND YEARS TO PLACE AN ORDER. DO NOT PROCEED WITHOUT THESE ITEMS.

** After you fill in the information and submit payment below, you will automatically be directed to our banner vendor (Military Tribute Banners) to upload your Honoree's picture and complete your order.

** Keep an eye out for an email from Military Tribute Banners and follow the instructions to approve or make changes to your banner. It will not be produced until you approve it.

* All banners will be shipped to and stored by Kiwanis Club of Lindenhurst for display by Memorial Day.

By submitting a Hometown Heroes banner order, you grant the Kiwanis Club of Lindenhurst, the Village of Amityville, the Village of Lindenhurst and the Town of Babylon permission to use the submitted photo (which includes a likeness of yourself or a friend or relative from whom you have obtained permission), the name and banner for display in the Hometown Heroes Banner Program and all promotional materials whether in print, digital media or otherwise. You further attest that all information provided with your submission is true and accurate and take full responsibility for its content.

The Kiwanis Club of Lindenhurst will replace banners that are lost or significantly damaged as a result of vehicle strikes at no cost to you. Banners that need replacement due to wear & tear from age, wind, sun, weather and other natural elements may incur a replacement fee of $50.
