    Please ensure you have the following documents readily available before you begin this application form, as you may need to refer to them. You will be asked to upload electronic (i.e. scanned) copies of these documents in the last section of this form. If you do not have access to electronic copies, you can provide printed copies to the school before your application progresses to an interview with the Principal. You will also need to pay an enrolment application fee after submitting the form, via EFTPOS at the office or over the phone. Please get in touch.

    Government regulations require schools to obtain immunisation records at the time of enrolment and keep records. School Health Services has to notified of children who are not immunised.

    Only an Australian Immunisation Register (AIR) Immunisation Status Report can be accepted.

    The Health Care Card Discount Scheme provides fee concession to the holders of eligible means-tested family concession cards.

    Consent and agreement are provided for the duration of the student’s enrolment. Any changes need to be notified in writing.

    As part of our school communication and engagement activities, our school takes photographs and videos of students for educational and community purposes. These may include use in classroom displays, school newsletters, website, social media platforms, Seesaw or other learning platforms, school yearbook, and promotional or community publications. The names of children are not published in social media, and only first names are used in the school newsletter. From time to time, Catholic Education WA may use an image for communication or promotional purposes. Only the school’s professional photography of class groups and individual student portraits is exempt from this permission.

    Your choice applies to ALL contexts and platforms (this includes Seesaw and the yearbook). No exemptions or partial permissions can be made for specific platforms or publications.

    YES – I give permission for my child to be photographed and/or recorded and for these images to be used by the school for educational and promotional purposes as outlined above.

    NO – I do not give permission for my child to be photographed or recorded in any context. I understand that this applies to all school settings and platforms, and I will explain to my child that they cannot be photographed at school.

    Children will have access to the Internet within the school guidelines and adequate supervision will always be available whilst using the Internet. If a child encounters any material on the web that may make them feel uncomfortable, they should minimise the screen and inform the teacher. The children will be advised to never give out personal information, including their phone number, last name or home address when using the Internet.
