Community Resource Connection Interest Application

Many who face food insecurity are also in need of additional support in areas such as housing, education, healthcare, workforce development, and family support. Our outreach partners are invited to attend food distributions and conduct community outreach to individuals and families receiving food assistance from the San Diego Food Bank (SDFB). Thank you for your interest in becoming an outreach partner of the Jacobs & Cushman San Diego Food Bank.

Please note the following:

· This is NOT an official partner application or a guarantee of partnership with the San Diego Food Bank. Please fill out and submit the following form. If it appears that your organization meets the initial eligibility criteria for partnership, you will be invited to apply for a potential partnership.

· Please keep in mind that your organization may not be approved to partner for a variety of reasons, including but not limited to: no active need for your service type at current food distribution sites; SDFB already working with other active outreach partners who provide the same or similar services; lack of capacity for outreach at distribution sites.

If your organization is not approved for outreach, please feel free to periodically check back in with the Community Resources department at SDFB.
