FUTSAL SELECT ID Session Tryout Registration Form

Our futsal select program is a competitive program open to both current SCPYS players and non-club players. There are two primary methods for being selected to our competitive program: first, players may be invited automatically, and second, they can attend a tryout and be selected based on their performance and team needs. Players will be selected by our SCPYS futsal director/coach. ALL OUR ID SESSIONS ARE FREE.
FUTSAL PROGRAM COST: $350

Open to all players (club / non-club)

(8 weeks of 1 hour training sessions 1x week) (2x futsal events)

- Includes futsal SCPYS Futsal jersey

9 players max per team

Teams will play in (2x) 5v5 futsal events

I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, the United States Soccer Federation, the State Association, the SCP Youth Soccer Program and or Club, the league and tournament, their associated directors, administrators, officers, managers, employees, coaches, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors, from any and all liability incurred in the conduct of, and my participation in, their soccer program(s). This includes owners, lessors, and lessees of premises, municipalities, government agencies, successors, heirs, and assigns.

I have completely read this document and fully understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily. My signature and or intitals/acceptance of this form attests to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, successors, and assigns.

For those individuals under the age of eighteen (18) years (minor):As the parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named above. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor in the execution of this Waiver of Liability and Release.

I grant to SCP Youth Soccer and its representatives and employees the right to take photographs of me and my Family in connection with the above-identified subject. I authorize SCP Youth Soccer, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that SCP Youth Soccer may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

A CLUB COACH WILL REACH OUT WITH A TRYOUT DATE ONCE REGISTRATION IS COMPLETE. PLEASE BRING SHINGUARDS,CLEATS AND BALL TO TRYOUTS.

Our Tryouts will be held in early December. The Program kicks-off in January of 2026

Our club will reach out in December ,once the outdoor Fall season has finished with a select date and time. SCPYS players are required to tryout as well. This is a seperate program not included with current curriculum.
