1. Boarding animals must be free of fleas and ticks or be treated at owner's cost.

2. There will be a daily charge for medication administration & kennel food added to boarding charges.

3. Owner authorizes Sea Island Animal Hospital to do whatever is necessary in case of illness or emergency situation, and at the discretion of the attending Veterinarian should medical care be needed, to include hospitalization.

4. Owner understands S.C. Law allows animals left more than (10) days beyond the agreed pick - up date to be considered abandoned and transferred to an appropriate animal shelter (R403;H4226)

5. Mandatory vaccinations to board include Rabies, Distemper/Parvo, Bordetella, Influenza and yearly fecal for canines. Rabies and Feline Distemper for felines.

If my pet is not current on vaccinations, an exam & vaccines will be given while boarding at owner's expense. I understand that vaccines should be given at least 7 days prior to boarding. Vaccines given on the day of admission to boarding may not fully protect my pet from infectious and contagious disease. I hereby waive and release kennel, its emplyees, and agents from any and all liability of any nature, for injury or damage which I or my dog may suffer as a result of lack of recommended vaccinations. I also accept full responsibility for any problems or illnesses this may cause for my pet(s).

6. Geriatric patients and/or patients with medical needs requiring special care may be subject to additional costs for services rendered in addition to their basic boarding fee.

7. Multiple pet boarding:
I understand that boarding multiple pets together may result in aggressive behavior. Dogs will be fed together unless otherwise alerted. I release all SIAH staff, owners and any representatives from any and all liability which I or my dog(s)/cat(s) may suffer including but not limited to injury, sickness, or death resulting from boarding together.

8. Bedding
To my knowledge my pet/pets do not have any history of chewing up beds and towels when left in cage. In the event of a medical emergency Sea Island Animal Hospital will perform the necessary procedures to save your pet. I release all SIAH staff, owners and any representatives from any and all liability which I or my dog(s)/cat(s) may suffer including but not limited to injury, sickness, or death resulting from boarding together.

9. Medical Illness
In the event of a medical illness, including but not limited vomit/diarrhea/anoresxia. I above for Sea Island Animal Hospital to treat my pet as deemed medically necessary if unable to contact me. I understand that I am responsible for additional fees.

10. I understand that boarding can sometimes be stressful. Should a veterinarian determine that my pet is showing signs of severe stress/anxiety or exhibiting signs of cage aggression; I agree to the use of anxiety and/or sedatives to make my pets stay more comfortable.

I understand that Sea Island Animal Hospital DOES NOT provide veterinary supervision overnight. I understand that my pet will be unattended if left overnight or boarding. I fully understand these risks and understand the veterinarians and hospital staff will try to minimize such risks. I will not hold Sea Island Animal Hospital, the veterinarians, or any staff member liable for any complications that may arise. I release SIAH from all liability should any accidents or injuries occur after hours.

If transfer to an 24hr emergency facility is recommeded; I undestand that Sea Island Animal Hospital does not offer transportation. I understand that I must arrange for my emergency contact or authorized person/persons to transport my pet.
