The purpose of the 10 minute Meet & Greet is to meet new students and families and to assess how well students are able to sing in tune. Very important in a choir setting!

Please provide details of any school-based music classes (including choir, instrumental and extra-curricular) your child may have had or is currently involved in and any instruments they may play. This is for teacher information to ascertain their level of music knowledge for planning.

I give permission for The Voice Studio Mackay to use video footage or photographs of my child. This usage may include all marketing channels online and offline. I agree that any images may be used by The Voice Studio Mackay for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information will not be used in conjunction with any images or video footage. I agree to the above photography & media consent.

I consent to my child participating in The Voice Studio Mackay classes, performances and activities. I understand that every effort will be made to contact me prior to any medical attention being given. If it is not possible to contact me, I hear by authorise The Voice Studio Mackay team to seek medical attention in the event of an accident, or medical mishap during my child's participation in the classes. I agree to the above health information.

I understand that there is an annual enrolment fee that covers public liability, administration, recital fees and sheet music resources, as well as a uniform for this class. I understand that an enrolment fee and class fees are non-refundable and non-transferable. I understand that if my child misses a group lesson, no make up lesson or refund will be provided. I understand that the enrolment fee is separate to the monthly class fees which are paid via automatic direct debit. I understand and agree to the above.

I understand that I will need to organise and pay for my child's costumes if required. Every effort will be made to keep this cost as low as possible.
