READ THIS ENTIRE DOCUMENT CAREFULLY BEFORE SIGNING. BY CHECKING THE BOX BELOW AND SUBMITTING THIS FORM, YOU ARE AGREEING TO RELEASE SHERWOOD ADVENTURE, LLC FROM LIABILITY AS DESCRIBED HEREIN. IF YOU DO NOT UNDERSTAND ANY PART OF THIS DOCUMENT, DO NOT SIGN IT.

PART 1: RELEASE AND WAIVER OF LIABILITY

SECTION 1: ACTIVITY AND PARTICIPANT IDENTIFICATION

This Agreement applies to participation in Archery Tag® events and related activities organized or hosted by Sherwood Adventure, LLC ("Company"). Archery Tag® is a competitive team sport in which participants shoot foam-tipped arrows using bows at opposing players and targets.

SECTION 2: SPECIFIC RISKS OF ARCHERY TAG®

I understand and acknowledge that participation in Archery Tag® involves inherent risks, including but not limited to the following:

• IMPACT INJURIES: Being struck by foam-tipped arrows or other projectiles traveling at speed, including impacts to the face, head, neck, torso, and limbs.

• EYE AND FACIAL INJURIES: Contact with projectiles or equipment to the eye, face, or head, including injuries occurring despite proper use of protective equipment.

• PHYSICAL CONTACT INJURIES: Collisions, trips, slips, or falls caused by contact with other players, obstacles, boundaries, or uneven playing surfaces.

• MUSCULOSKELETAL INJURIES: Strains, sprains, fractures, or other injuries from drawing bows, rapid movement, pivoting, or sudden changes in direction.

• EQUIPMENT-RELATED INJURIES: Injuries caused by malfunction, misuse, or failure of bows, arrows, protective masks, netting, or other equipment.

• ENVIRONMENTAL CONDITIONS: Injuries resulting from outdoor heat, sun exposure, uneven terrain, or other conditions at the event venue.

• OVEREXERTION: Physical exhaustion, heat-related illness, dehydration, or cardiovascular stress from vigorous activity.

• ACTIONS OF OTHER PARTICIPANTS: Injuries caused by the intentional or unintentional acts or omissions of other players, spectators, or bystanders.

• OTHER UNFORESEEN RISKS: Risks not currently known or foreseeable that are inherent to the nature of the activity.

I understand that serious bodily injury, including permanent disability or death, may result from participation. I acknowledge that no amount of instruction, supervision, or protective equipment can eliminate all risk.

SECTION 3: ASSUMPTION OF RISK

I represent that I am in good health and physically capable of participating in Archery Tag®. I have disclosed any medical conditions that may affect my ability to safely participate. I voluntarily and knowingly assume all risks associated with participation in Archery Tag®, including the specific risks described in Section 2 above, whether those risks arise from the negligence of the Company or from any other cause. I accept full responsibility for any injury, loss, or damage I may sustain.

Note: Pursuant to Article 18, Section 5 of the Arizona Constitution, assumption of risk is a question of fact to be determined by a jury. This acknowledgment reflects my knowing and voluntary acceptance of risk and is not intended to operate as a complete bar to recovery in all circumstances.

SECTION 4: RELEASE AND WAIVER OF LIABILITY

In consideration of being permitted to participate in Archery Tag® activities, I hereby RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Sherwood Adventure, LLC, its owners, members, administrators, directors, officers, agents, volunteers, employees, other participants, sponsors, advertisers, and owners or lessors of premises where the activity takes place (collectively, the "Released Parties"), from any and all claims, demands, losses, or damages arising out of or related to my participation in Archery Tag®, including claims based on ordinary negligence of the Released Parties.

THIS RELEASE DOES NOT APPLY TO CLAIMS ARISING FROM GROSS NEGLIGENCE, WILLFUL MISCONDUCT, OR INTENTIONAL ACTS OF THE RELEASED PARTIES. Nothing in this Agreement is intended to, nor shall it, release any Released Party from liability for conduct that Arizona law does not permit to be waived.

SECTION 5: INDEMNIFICATION

I agree to indemnify and hold harmless each of the Released Parties from any loss, liability, damage, cost, or expense (including reasonable attorneys' fees) they may incur as a result of any claim brought by me or on my behalf arising from my participation in Archery Tag®, to the extent such claim is inconsistent with this Agreement.

SECTION 6: SAFETY RULES AND CONDUCT

I agree to:

• Follow all safety rules, instructions, and directions provided by Sherwood Adventure, LLC staff.
• Wear all required protective equipment, including facial/eye protection, for the duration of play.
• Immediately discontinue participation and notify staff if I believe conditions are unsafe, I feel unwell, or I observe a safety hazard.
• Conduct myself in a sportsmanlike manner and refrain from conduct that endangers other participants.

SECTION 7: EMERGENCY MEDICAL AUTHORIZATION

In the event of an injury or medical emergency arising from my participation, I authorize Sherwood Adventure, LLC and its staff to seek or facilitate emergency medical treatment on my behalf. I understand that the Company is not responsible for the cost of such treatment. I represent that I do not have any medical conditions that would prevent participation, or if I do, I have consulted a physician prior to participation.

PART 2: MEDIA AND TALENT RELEASE

This section is a separate agreement from the liability waiver above. Please read it independently before agreeing.

SECTION 8: GRANT OF RIGHTS

I grant Sherwood Adventure, LLC and its designees the right to record, photograph, and use my name, likeness, image, voice, appearance, and performance ("Content") captured during Archery Tag® events, whether on videotape, film, photographs, audio recordings, digital media, or any other format now known or later developed.

SECTION 9: SCOPE OF USE

This grant includes the right to:

• Edit, adapt, mix, or duplicate the Content in whole or in part.
• Broadcast, exhibit, publish, market, or distribute the Content via television, internet, social media, home video, theater, or any other platform.
• Use the Content for commercial or non-commercial promotional, marketing, or advertising purposes.

SECTION 10: OWNERSHIP AND COMPENSATION

Sherwood Adventure, LLC shall have complete ownership of the Content, including all copyright interests. I acknowledge that I have no ownership interest in the Content and will receive no financial compensation for its use. The rights granted herein are perpetual and worldwide.

SECTION 11: CLEARANCES AND RELEASE

I confirm that I have the right to enter into this Agreement and am not restricted by any commitments to third parties. I hereby release and indemnify Sherwood Adventure, LLC and its officers, employees, agents, and designees from any and all claims arising out of the authorized uses described above.

PARENT / LEGAL GUARDIAN CONSENT (PARTICIPANTS UNDER AGE 18)

━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━

If the participant is under 18 years of age, a parent or legal guardian must complete this section.

I represent that I am the parent or legal guardian of the minor named on this form and that I have the legal authority to execute this Agreement on the minor's behalf. I have read and understood this Agreement in its entirety. I understand the nature of Archery Tag® and the specific risks described above, and I believe the minor to be qualified and physically capable of participating safely.

On behalf of the minor, I hereby RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND HOLD HARMLESS each of the Released Parties from all claims, demands, losses, or damages arising from ordinary negligence in connection with the minor's participation in Archery Tag®. I agree that if, despite this release, the minor or anyone acting on the minor's behalf brings a claim against any Released Party, I will indemnify and hold harmless each Released Party from any resulting litigation expenses, attorneys' fees, loss, liability, damage, or cost.

I also consent to the use of the minor's name, likeness, image, and performance in accordance with the Media and Talent Release set forth in Sections 8–11 above.

GOVERNING LAW

This Agreement is governed by the laws of the State of Arizona. If any provision of this Agreement is found to be unenforceable, the remaining provisions shall continue in full force and effect.
