Client-Counselor Service Agreement
Welcome to Ree Vision Network. This document contains important information about our professional services and business policies. It also contains summary information about the Health Insurance Portability and Accountability Act (HIPAA), a federal law that provides privacy protections and patient rights about the use and disclosure of your Protected Health Information (PHI) for the purposes of treatment, payment, and health care operations. Although these documents are long and sometimes complex, it is very important that you understand them. When you sign this document, it will also represent an agreement between us. We can discuss any questions you have when you sign them or at any time in the future.
Goals of Counseling
Counseling will be goal-driven, and the client will set the goals (if counseling is court-ordered, the court will have input on goals). Goals can be long-term (i.e., improving quality of life) or more immediate (decreasing anxiety/depression).
Risks/Benefits of Counseling
It is important to understand that while counseling has many benefits (develop coping skills, make positive changes, reduce mental health symptoms, etc.), there are some risks. Unpleasant memories could come to the surface, difficult topics may be discussed, progress can happen slower than desired, improvements can be followed by regressions, and there are no guarantees counseling will work for you.
Appointments
Appointments will ordinarily be 45-60 minutes in duration, once per week at a time we agree on, although some sessions may be more or less frequent as needed. If you need to cancel or reschedule your session, please provide 24 hours' notice or a no-show fee may be assessed. In addition, you are responsible for coming to your session on time; if you are late, your appointment will still need to end on time.

Record Keeping
All paper records are kept locked up in a file cabinet. ReeVision Network has electronic medical records (encrypted and password-protected) to include daily notes and treatment goals. Records will be kept for at least 7 years but may be kept for longer.
Professional Fees/Insurance
You are responsible for paying at the time of your session unless prior arrangements have been made. Payment must be made by check, cash, or credit/debit card. If you refuse to pay your debt, we reserve the right to use an attorney or collection agency to secure payment. If you anticipate becoming involved in a court case, we recommend that we discuss this fully before you waive your right to confidentiality. If your case requires my participation, you will be expected to pay for the professional time required. Fees are non-negotiable. There is a discount for self-pay clients. Fees are subject to change at counselor's discretion. For insured clients, the insurance company will be billed, and the client will be responsible for any co-payments or deductibles. For billing purposes, this practice must disclose the dates of service and diagnosis code(s). By signing this agreement, you agree that I can provide requested information to your insurance plan for payment reimbursement. Further, although it is rare, insurance companies may choose your file to audit. If so, we will notify you prior to this occurring. It is the client's responsibility to ensure that services are covered by their insurance plan; if they are not covered, the client is responsible for the full payment. In addition, the client is responsible for obtaining authorization for services. The client is responsible for notifying the counselor if there is any change in insurance or coverage.

We are a small practice and don't currently have an administrative assistant. Therefore, we are often not immediately available by telephone. You may leave a message on our confidential voicemail, and we will make every attempt to return calls within 24 hours. If you feel you cannot wait for a return call or if it is an emergency situation, go to your local hospital or call 911. You can also reach us by email; please request individual email addresses.
Credentials
At times we offer counseling and psychological testing services provided by a Licensed Professional Counselor or Residents in Counseling. Residents are not Licensed Professional Counselors. However, they are registered with the Virginia Board of Counseling to work under the supervision of a Board-approved Supervisor to provide counseling services while they work towards obtaining a license. If you work with a Resident, understand that the Resident's Supervisor assumes full responsibility for the clinical activities of the Resident for the duration
