This form is here to assist you in compiling and providing us with all of the basic information that we need to complete your company or trust or partnership tax return accurately and efficiently. This form is not a substitute for completing and preparing the income tax return.
INSTRUCTIONS:

1. Please complete this form in one sitting as you cannot save and return. We recommend reviewing all the questions first to indicate the necessary information to prepare or gather.

2. We request naming documents before uploading them to include the corresponding question number or indicator that is relevant to your return together with a description of the details. For example a bank statement would be named 2.1 ANZ account#1234. Documents can be saved as either PDF, word, or excel.

3. Please respond to questions that are relevant to your circumstances and follow the prompts.
