When performed by a qualified structural osteopath, osteopathic manipulation and electro therapy is an effective and safe method of treatment for many musculoskeletal conditions.
However, there are risks associated with any treatment and we are required to inform you of these.
Please read the following carefully and discuss any questions you may have with your Osteopath. If you agree with the following, sign and return this form to your Simplybe.
I request and consent to the following techniques: Massage, Electro therapy and Structural osteopathy.
I confirm that I have had the opportunity to discuss the proposed treatment with the osteopath. I understand that results are not guaranteed. I have been informed that, in the practice of osteopathy, as in the practice of medicine or other manual therapies, there are some very slight risks including, but not limited to, muscle and joint soreness, muscle strain, joint strain, fractures and disc injuries.
I do not expect the osteopath to be able to anticipate and explain all of the risks I accept that the osteopath treating is exercising his or her judgment during the course of my treatment in such a manner and to the extent that he or she feels best at the time, based on the facts then known, and in my best interests.
This clinic has a 24 hour cancellation policy that applies to all appointments. Failure to provide 24 hours’ notice when changing or cancelling appointment as well as missed appointments will result in being charged at the full appointment fee.
