    Financial Aid Appeal Form 2022-2023

    Financial aid requests are considered for unusual circumstances causing financial hardship. This form gets the conversation started. Additional documents in support of your circumstance will be required. Appeal requests are reviewed on a case-by-case basis.


  • Please come into the Office of Financial Aid and ask to talk to a financial aid counselor. The office is open from 8 am to 5 p.m. Monday through Friday. It is located on the middle floor of Christy Administration Building.

  • Suggested documents:

    • Termination Letter
    • Statement of Unemployment Benefits
    • If a new job exists, a recent paystub from new job

  • Suggested document:

    • Divorce Decree

  • Suggested document:

    • Statement of what the income was and how it was utilized

  • Suggested document:

    • 1040 Schedule A or proof of medical expenses

  • Signature

  • Warning! If you purposely give false or misleading information to help establish eligibility for federal student aid, you may be subject to a $20,000 fine, a prison sentence, or both.  By signing this Verification Statement, we certify that all the information reported in support of the student’s application for financial aid is complete and accurate.  Please be prepared to supply additional documentation if requested by the Financial Aid Office.
